Hospital Quality Transparency Dashboard

Hospital Quality Institute, in cooperation with the Patient Safety Movement Foundation and California Hospital Association, has created dashboards of publicly-available quality data for each California Hospital Association short-term, acute-care member hospital. These dashboards provide information on eight measures. Hospitals may also add optional comments about performance and initiatives at that hospital. HQI encourages each hospital to publish the information in the dashboard to its public-facing website to increase hospital quality transparency.

HQI supports and facilitates hospital leadership in providing increased quality transparency and subsequent quality improvement. Our statewide goal is to have a Quality Transparency Dashboard or its equivalent published on every California hospital’s website by the end of 2018.

Quality Transparency Dashboard Sepsis Mortality Calculation (Update Jan 2019)

How does HQI calculate sepsis mortality?  (ICD-10-CM)

ICD-10-CM Codes used to identify sepsis cases and calculate the sepsis in-hospital mortality rate for inpatient discharges as presented in the Quality Transparency Dashboard.  While sepsis is diagnosed, based on codes, it is not strictly the identified cause of death. The modified Dombrovskiy Method for sepsis case identification follows this process:

1. What is the Quality Transparency Dashboard? 

The Quality Transparency Dashboard is an Excel file consisting of a single presentation page and two supporting tabs. The presentation page is a succinct expression of five quantitative measures and three programs. The dashboard includes consumer-level explanations and limitations of each measure.
