HCAHPS Hospital Compare Star Ratings


The HCAHPS survey results have been publicly reported on Hospital Compare since 2008.  On April 16, 2015, the Centers for Medicare & Medicaid Services (CMS) introduced a star rating system for hospital performance on select quality metrics with the goal of making scores simpler for consumers to interpret.  The Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS) metrics were among the measures selected for publication on the Hospital Compare website as a star rating system.

Twelve HCAHPS star ratings appear on Hospital Compare, one for each of the eleven HCAHPS measures as well as a new Summary Star Rating, which is a simple average of the eleven HCAHPS star ratings. Ratings range from one to five stars, with five stars being the best. Hospitals are not granted half stars or quarter stars.

Hospital Quality Institute (HQI) and California Hospital Association (CHA) have joined others in raising concerns that simplified rating systems, such as star ratings, have the potential to misrepresent the complexity of health care. Additional concerns include the lack of clarity on how CMS will adapt and update star ratings as various measures change, which could create confusion for patients and hospitals. CMS’ methodology to convert HCAHPS survey data into star ratings is available on the HCAHPS website at http://www.hcahpsonline.org/StarRatings.aspx. HQI and CHA welcome hospital input and feedback on HCAHPS methodology, including the new star ratings.
