Webinar: Partners in Safety- Engaging Patients to Promote Safe Medication Use
Patient safety awareness week celebrates providing safe and effective care to our patients. Patients are a key player in the health care team and in the provision of safe patient care. This webinar will focus on the importance of engaging patients when it comes to using medications safely. The goal is to highlight and promote methods of integrating the patient in medication use to enhance safety.

Katayoon Kathy Ghomeshi is a Medication Safety Officer at UCSF Medical Center and a Health Sciences Assistant Clinical Professor at UCSF School of Pharmacy.
Dr. Ghomeshi holds dual board certification as a Board Certified Pharmacotherapy Specialist as well as a Certified Professional in Patient Safety. Dr. Ghomeshi is a graduate of the University of Maryland School of Pharmacy, she completed a post-graduate residency in Managed Care pharmacy at Kaiser Foundation Hospitals/Kaiser Permanente. She continued on to complete a post-graduate specialty residency in Medication-Use Safety at the Johns Hopkins Hospital, during which time she trained at the Institute for Safe Medication Practices, the American Society of Health-System Pharmacists, the Johns Hopkins Armstrong Institute for Patient Safety and Quality, as well as the Maryland Patient Safety Center.
Dr. Ghomeshi currently serves on a number of appointed roles in national and state committees. She is Vice Chair of the American Society of Health-System Pharmacists (ASHP) Section of Inpatient Care Practitioners’ Advisory Group on Medication Safety, and Vice Chair of the California Society of Health-System Pharmacists Council on Professional Affairs. She is also an appointed member of the Board of Pharmacy Specialties (BPS) Specialty Council, the California Hospital Association (CHA) Medication Safety Committee, and the California Society of Health-System Pharmacists (CSHP) Healthcare Reform Committee.
Continuing Education Credit
One complimentary CE unit will be provided to dues-paying member hospitals of the Hospital Quality Institute, CHPSO Patient Safety Organization, the California Hospital Association or the regional hospital associations in CA.
HQI is an approved continuing education (CE) provider by the California Board of Registered Nursing and will provide CHPSO members an opportunity to earn CEs. Provider Number CEP16793 for 1.0 contact hour.
HQI does not currently offer ACPE approved Pharmacy CE, but the State of California will accept CE from courses which have been approved for continuing education by the Medical Board of California, the California Board of Podiatric Medicine, the California Board of Registered Nursing or the Dental Board of California towards their California continuing education requirement (Section 1732.2(b) of the California Code of Regulations). Pharmacists need to keep track of CE from these sources on their own as they are not submitted to NAPB.
